- We approach your healthcare needs with a Health Care Team approach so you may see any of our providers.
- Please arrive 10 minutes prior to your appointment time to complete any required paperwork.
- We do take Walk-In and Same-Day appointments and these will be worked in around the scheduled and pediatric patients.
- Please bring your insurance card, identification (drivers license, ID card), and medication list with you to your appointment.

- Please be prepared to pay your co-pay before you are seen. If you are Self Pay, Dr. Geyman offers a generous discounts if paid in full at time of visit.
- If your insurance (including Medicaid) requires you to designate a PCP please contact them prior to your appointment to change us to your PCP. If this is not done you may get a bill for the full cost of visit and treatment given.
- If you have a work related injury, please bring the appropriate paperwork with you to the visit including the contact person for your employer.
- If you need a medication refill for a medication that we prescribed please contact your pharmacy for them to submit a refill request. Please allow 48-72 hours for it to be processed. If you have zero refills your Health Care Team will review to see if they will elect to refill the prescription. You will be required to have an appointment if: we have not prescribed the medication or are required by regulations to see you before it can be filled.
- If you have seen another provider, specialist, emergency room/urgent care or have been hospitalized please inform our front desk team. That way, we can assist you in requesting copies of your records so we can continue to have a comprehensive understanding of your health to better serve you and your specific needs.
