Please send an electronic copy or photo with both sides of your insurance card via email to riverstonebilling@gmail.com prior to your appointment.
- Please include the following:
- Subject: BFFM Patient (name)
- Body of email:
- Patient of BFFM
- Names & Date of Birth for ALL PATIENTS covered under this policy
- Attach front & back of Insurance card(s)
- Attach a copy of ID (drivers license) of primary policy holder and any adult patients.
- Effective date of coverage (if known)
