Please bring completed forms into the office. 

If you have an electronic copy of your insurance card please send an email to riverstonebilling@gmail.com prior to your appointment. Please include the following:

• Subject: BFFM Patient (name)
• Body of email:
  • Patient of BFFM
  • Names & Date of Birth for ALL PATIENTS covered under this policy
  • Attach front & back of Insurance card(s)
  • Attach a copy of ID (drivers license) of primary policy holder and any adult patients. 
  • Effective date of coverage (if known)
